What you should know
- We'll ask about your height, weight, wellness, fitness habits, nutrition, weight-loss experience, goals, and state of residence. Some of this is consumer health data, so we ask for your permission before collecting it.
- We use your answers to calculate your preliminary BMI, personalize your experience, determine which oMeds pathway is available, and operate, secure, and improve the experience using aggregated or de-identified information.
- We do not sell your individual questionnaire health data, use your individual answers for targeted advertising, or provide your individual questionnaire answers to Crunch Fitness for advertising, member targeting, or medical decision-making.
- Deleted within 90 days. Raw questionnaire responses are deleted or de-identified, and you may request deletion sooner as described in our Consumer Health Data Privacy Notice.

Privacy & consent details
What we collect
Height and weight, which we use to calculate a BMI on our servers.
Basic health history relevant to weight management, including relevant conditions, medications, and prior GLP-1 use.
The U.S. state you live in, so we can check whether the program operates there.
We do not ask for your name, email address, or phone number during this assessment.
Why we collect it
To calculate your preliminary BMI, personalize your experience, and determine which oMeds pathway is available to you.
To operate, secure, and improve the experience, using aggregated or de-identified information.
This assessment is not a medical evaluation, not a diagnosis, and not a determination that treatment is appropriate for you. Only a licensed provider makes that determination, in a live consultation.
Who can see it
Authorized oMeds personnel who operate and support the program.
Service providers that host and secure our systems, under contract and only on our instructions.
Crunch Fitness does not receive your individual questionnaire answers or your individual BMI. Crunch Fitness may receive aggregated or de-identified program-level insights that do not identify you.
We do not sell your individual questionnaire health data, and we do not share it with advertising networks or data brokers, or use your individual answers for targeted advertising.
How long we keep it
Assessment answers are automatically deleted or de-identified within 90 days.
If you go on to purchase, your clinical record is kept separately under the applicable medical-records requirements described in the Privacy Policy.
Your choices and rights
You can decline. Declining does not affect your Crunch Fitness membership, and you can still browse oMeds.
You can withdraw your consent at any time, which stops further processing of your assessment.
You can request access to, or deletion of, the health data collected here.

How it's protected
Your answers are transmitted over an encrypted connection and stored encrypted at rest.
Access is restricted, logged, and limited to personnel who need it.
