Your provider may use and disclose your protected health information for treatment, payment, and healthcare operations as permitted by HIPAA. Additional uses and disclosures may require your written authorization.
Your rights
- Request access to and copies of your medical record.
- Request an amendment to your record if you believe it is incomplete or incorrect.
- Request an accounting of certain disclosures of your PHI.
- Request restrictions on certain uses and disclosures.
- Request confidential communications by alternative means or at alternative locations.
- File a complaint with your provider, with us, or with the U.S. Department of Health and Human Services without retaliation.
