Telehealth Consent & Disclosure

What telehealth care looks like through our network, your rights as a patient, and the limits of remote care.

§01

Nature of telehealth care

Telehealth means clinical care delivered remotely using secure live video and voice instead of an in-person visit. Through our platform you may interact with a licensed provider for evaluation, prescription review, follow-up, lab review, and ongoing monitoring without traveling to a physical clinic.

Telehealth has both benefits (convenience, continuity, faster follow-up) and limitations (no hands-on physical exam, dependence on the technology working, and certain conditions that genuinely require in-person care).

Licensed-provider network

Clinical services are delivered by independent licensed providers — physicians (MD or DO), nurse practitioners (NP), and other clinicians — who are credentialed in the states where they provide care. oMeds is not a pharmacy and we do not employ providers; we coordinate the platform, intake, scheduling, lab logistics, and patient experience that surround the clinical relationship between you and your provider.

State availability

Care is only available in states where a network provider is currently licensed and authorized to practice. Eligibility is confirmed during intake based on your residence and the state where you will be located when receiving care. If we cannot serve your state at the time of intake, we will tell you and will not bill you for clinical services that were not delivered.

Your rights

You have the right to:

  • Decline telehealth and request an in-person referral at any time.
  • Withdraw consent for telehealth and discontinue services without penalty beyond fees for care already delivered.
  • Request a different provider within the network, subject to availability.
  • Request copies of your medical records held by your treating provider.
  • Be informed of any limitations on the services you can receive remotely.

Records and recordings

Visit summaries, prescriptions, lab results, secure messages, and clinical notes are stored in your provider's record system in accordance with applicable privacy law (including HIPAA where applicable). Visits may be recorded only when required by law or with your explicit consent. Recordings made by patients of clinical visits require the provider's prior agreement.

Technology limitations

Telehealth depends on functional internet, devices, and software. Audio or video quality issues, dropped connections, or platform outages can interrupt care. If a telehealth visit is interrupted and cannot be re-established, your provider will reschedule the visit.

Telehealth is not a substitute for in-person care when an in-person exam, procedure, imaging, or hands-on diagnostic is clinically required. Your provider will tell you if in-person care is needed.

Prescribing limits

Providers in our network prescribe only when clinically appropriate based on evaluation, history, and labs. Not every patient who requests a therapy will be prescribed it. Controlled substances are prescribed only in compliance with the federal Ryan Haight Act and applicable state laws — see our Ryan Haight Act policy for details.

Consent acknowledgment

By proceeding with intake, scheduling a visit, or accepting clinical services through this platform, you acknowledge that you have read this disclosure, understand the nature and limitations of telehealth care, and consent to receive care via telehealth from a licensed provider in our network. You may withdraw this consent at any time by notifying your provider or our care team in writing.

Emergencies are not handled through telehealth

If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
