ApoB measures the total number of atherogenic lipoprotein particles (LDL, VLDL, IDL, Lp(a)), each of which carries exactly one ApoB molecule. Elevated ApoB therefore reflects a high particle count regardless of how much cholesterol each particle carries — and it is increasingly recognized as the more informative cardiovascular risk metric than LDL-C alone. Drivers include genetics (familial hypercholesterolemia, ApoE4 variants, elevated Lp(a)), insulin resistance and visceral adiposity (which raise VLDL production), hypothyroidism (which slows LDL receptor activity), certain diets, and reduced hepatic clearance with age.
