Both matter, but ApoB is increasingly recognized as the more informative metric. Every atherogenic lipoprotein particle — LDL, VLDL, IDL, Lp(a) — carries exactly one apolipoprotein B molecule. ApoB therefore counts the total number of these particles directly. LDL-C, by contrast, measures only the cholesterol cargo carried inside the LDL particles. The two usually correlate, but they diverge meaningfully in insulin resistance, metabolic syndrome, and small-dense-LDL patterns — situations where many cholesterol-poor particles can carry less total cholesterol while still presenting many atherogenic particles to the artery wall. In that discordance, ApoB more accurately reflects cardiovascular risk than LDL-C.
