Every atherogenic lipoprotein particle in the bloodstream carries exactly one ApoB molecule on its surface. Measuring ApoB therefore counts particles — LDL, VLDL, IDL, and Lp(a) — directly. LDL cholesterol, by contrast, measures the cholesterol cargo inside those particles, which can underestimate particle burden in patients with small dense LDL, insulin resistance, or elevated triglycerides. The published cardiovascular literature consistently shows ApoB outperforming LDL cholesterol as a predictor of atherosclerotic events at the population level, and modern preventive cardiology treats ApoB as the primary lipid endpoint. ApoB is read in context with Lp(a) (a largely genetic, independent risk factor), triglycerides, HDL, and fasting insulin to build the full atherogenic-particle picture.
