Lipoprotein(a) is an LDL-like particle with an attached apolipoprotein(a) tail. Its level is roughly 80–90% genetically determined, established in early life, and largely unchanged by diet, exercise, or standard lipid-lowering therapy. Elevated Lp(a) independently predicts atherosclerotic cardiovascular disease and aortic valve stenosis. Because Lp(a) is essentially fixed, it is checked once in adulthood (sometimes twice in a lifetime) and then used to calibrate the intensity of every modifiable cardiovascular lever — ApoB, blood pressure, glycemic control, smoking cessation, and where indicated, lipid-lowering therapy. A normal Lp(a) does not eliminate risk; an elevated Lp(a) elevates the bar for everything else. Provider review reads Lp(a) within the broader cardiometabolic panel rather than in isolation.
