Enclomiphene is the trans-isomer of clomiphene used off-label to restore endogenous testosterone production while preserving fertility. It is an alternative to TRT for select candidates who want to maintain testicular function.
Selectively blocks estrogen feedback at the hypothalamus and pituitary, increasing endogenous LH and FSH secretion and restoring native testicular testosterone production.
What you need to know
Enclomiphene works upstream of the testes by blocking estrogen-mediated negative feedback at the hypothalamus and pituitary. LH and FSH rise, stimulating native testosterone production and preserving spermatogenesis. Unlike exogenous testosterone, enclomiphene typically does not suppress fertility. Candidacy depends on intact testicular function (confirmed via FSH/LH/inhibin context), fertility goals, and provider judgment.
