Non-HDL cholesterol vs LDL cholesterol — what's the difference?

LDL cholesterol measures one atherogenic fraction; non-HDL captures all atherogenic cholesterol (LDL plus VLDL plus remnants). Non-HDL is preferred when triglycerides are high.

LDL cholesterol — directly measured or calculated by the Friedewald equation — captures the cholesterol in low-density lipoprotein particles. Non-HDL cholesterol, calculated as total cholesterol minus HDL, captures LDL plus VLDL plus remnant cholesterol — all atherogenic fractions. When triglycerides exceed 200 mg/dL, calculated LDL-C becomes unreliable and non-HDL is preferred. Within oMeds review, both are reported on every lipid panel and interpreted alongside ApoB.
