TRT monitoring follows a defined cadence. Baseline labs establish total and free testosterone, SHBG, estradiol, hematocrit/CBC, lipids, CMP, and PSA (in men >40). The first recalibration is at 8-12 weeks post-initiation and reads the same panel plus dose-response. Once dose is stable and biomarkers are within target, cadence moves to every 3-6 months. Hematocrit and PSA monitoring continue indefinitely. Provider review responds to specific triggers (estradiol drift, hematocrit approaching 52%, PSA velocity) with dose, modality, or adjuvant adjustments.
