In men, estradiol is produced primarily by peripheral aromatization of testosterone via the enzyme aromatase, which is concentrated in adipose tissue. Elevated estradiol therefore typically reflects one or more of: higher body fat (more aromatase activity), supraphysiologic exogenous testosterone dosing (more substrate), alcohol use (which both stimulates aromatase and impairs hepatic estrogen clearance), certain medications, liver dysfunction, hCG use, and — rarely — estrogen-producing tumors. Symptoms can include water retention, breast tenderness, mood changes, and reduced libido despite adequate testosterone. Management is rarely 'crush estradiol with an aromatase inhibitor.'
