Fasting glucose reflects the balance between hepatic glucose output overnight and insulin-mediated suppression of that output. Elevated fasting glucose almost always indicates that this regulatory loop is impaired — insulin resistance has progressed far enough that the pancreas can no longer fully restrain hepatic glucose production. Contributors include visceral adiposity, low skeletal-muscle mass, chronic caloric surplus, poor sleep (one night of restricted sleep measurably reduces insulin sensitivity), the dawn phenomenon (early-morning cortisol- and growth-hormone–driven hepatic glucose release), certain medications (glucocorticoids, some atypical antipsychotics), and the natural progression toward prediabetes and type 2 diabetes. Fasting glucose is read alongside fasting insulin and HOMA-IR for a complete picture — because insulin resistance precedes glucose elevation by years.
