Fasting insulin and hemoglobin A1c answer different questions. Fasting insulin reflects how hard the pancreas is working to keep glucose normal — it rises early in insulin resistance, often years (sometimes a decade) before A1c crosses any diagnostic threshold. A1c reflects average glycemia over the prior ~3 months and is the standard for diagnosing prediabetes and type 2 diabetes — but by the time A1c is elevated, the metabolic dysfunction has typically been progressing silently for a long time. For early detection in asymptomatic patients, fasting insulin (or, more precisely, the HOMA-IR derivation combining it with fasting glucose) is more sensitive. For diagnosis and ongoing monitoring of established dysglycemia, A1c remains the standard.
