LDL particle number (LDL-P) and ApoB both quantify atherogenic particle burden and correlate strongly, but they measure different things. LDL-P counts LDL particles directly by NMR spectroscopy and provides subfraction detail (sdLDL). ApoB counts the apolipoprotein B-100 molecule, of which each LDL, VLDL, and remnant particle carries exactly one. ApoB is more standardized across labs and is endorsed by major guidelines. Within oMeds review, ApoB is the default; LDL-P is used in advanced workups requiring subfraction context.
