Yes — typically. Exogenous testosterone generally lowers SHBG, often by 20–40% depending on modality, dose, and baseline. Injectable testosterone tends to suppress SHBG more than transdermal forms at equivalent total testosterone exposure. Because SHBG binds testosterone, a lower SHBG means a higher free (bioavailable) fraction of any given total testosterone — which is why two men with the same total testosterone can feel very different on therapy. Practical implications: a patient whose SHBG has dropped sharply may have higher functional androgen activity than the total testosterone number suggests, and a dose adjustment may be reasonable.
