Low SHBG (sex hormone-binding globulin) is rarely a primary problem on its own — it is a signal. SHBG is produced by the liver and its concentration is sensitive to metabolic and hormonal inputs. Low SHBG is most commonly seen with insulin resistance, central obesity, hypothyroidism, NAFLD, exogenous androgen use, and certain medications. Mechanistically, low SHBG means a higher fraction of total testosterone is unbound (free) and bioavailable — which on paper sounds favorable but in context typically reflects an unfavorable metabolic backdrop. It also means total testosterone reads can underestimate androgen activity.
