Why am I gaining weight in the first weeks of testosterone optimization?

Short answer

Early weight gain on TRT is usually water retention and lean-mass gain, not fat gain. It typically stabilizes over 4–8 weeks. Estradiol, sodium balance, and training stimulus shape the experience.

Full answer

Early scale weight on TRT is dominated by intracellular water retention and increases in lean tissue, not fat accumulation. The shift is driven by androgen and estradiol effects on sodium handling and protein synthesis. It typically stabilizes within 4–8 weeks. Body composition (DEXA or measured circumferences) and biomarkers are better signals than scale weight. Symptomatic edema, breast tenderness, or rapid weight gain prompts a review of dose, modality, frequency, and estradiol. Sustained scale gain beyond the early window prompts a metabolic review (sleep, nutrition, training, alcohol).
