Genitourinary syndrome of menopause includes vaginal dryness, dyspareunia, and urinary changes. Local vaginal estradiol is highly effective and has a different systemic risk profile than oral estrogens; it is discussed within BHRT review. Systemic BHRT is considered when vasomotor, sleep, mood, and cognitive symptoms are also present and biomarkers match. Cardiovascular, thrombotic, and breast history are reviewed before initiation.
