Unlike menopause, testosterone decline in men is gradual rather than abrupt. Symptoms — fatigue, reduced libido, mood change, loss of lean mass, reduced exercise tolerance — overlap with many other conditions. Diagnosis requires two morning total testosterone measurements below the reference range with consistent symptoms, supplemented by free testosterone, SHBG, LH, FSH, prolactin, and thyroid status.
