Hormone optimization is read against a four-panel standardized biomarker baseline: Hormones (total testosterone, free testosterone, SHBG, estradiol, LH, FSH, and — where indicated — progesterone, DHEA-S, prolactin, thyroid), Lipids (total cholesterol, LDL, HDL, triglycerides, ApoB where indicated), CBC (with hematocrit and platelets monitored throughout TRT), and CMP (liver enzymes, kidney function, glucose, electrolytes). The panel is required at baseline before TRT or BHRT initiation and on the provider-defined recalibration cadence thereafter. The clinical purpose is not 'show numbers.' It is: read the patient against their own baseline, watch trajectory, surface drift early, and adjust modality or dose before symptoms regress. For all non-hormone pathways (NAD+, peptides, sexual wellness, hair restoration, metabolic), the biomarker panel is optional — included when the patient or provider requests it — and never required.
