Vitamin D and vitamin K2 do different things, and the case for considering them together rests on the calcium-trafficking story. Vitamin D drives intestinal calcium absorption — making sure enough calcium reaches the bloodstream. Vitamin K2 activates two proteins, osteocalcin and matrix Gla protein, that respectively direct circulating calcium into bone matrix and prevent its deposition into arterial walls. The mechanistic logic — that high-dose vitamin D without adequate K2 could theoretically promote vascular calcification — is plausible, and observational data are suggestive. The randomized trial evidence for routine co-supplementation in otherwise healthy adults is not yet definitive. Provider-reviewed wellness individualizes the decision based on vitamin D dose, dietary K2 intake (fermented foods, certain cheeses), cardiovascular risk, and whether the patient is supplementing calcium.
