Common, reversible TRT side effects include acne, transient water retention, mild estradiol elevation, hematocrit drift, unmasking of obstructive sleep apnea, and injection-site reactions where applicable. Less common: gynecomastia (estradiol-driven), mood lability, lipid shifts, and rare hepatic effects with oral routes. Management is largely about dose, modality, and frequency rather than additional medications. Aromatase inhibition is used sparingly. Hematocrit is monitored at every recalibration.
