Androgenetic alopecia is driven by genetic sensitivity of the hair follicle to dihydrotestosterone (DHT), the 5-alpha reductase product of testosterone. Raising testosterone on TRT raises DHT proportionally and, in genetically predisposed men, can accelerate patterned hair loss. The risk is individual: men with no family history and no current shedding rarely notice change, while men already losing hair often see acceleration.
