Bioidentical hormone optimization is considered safe for most candidates when initiated against the standardized biomarker panel, dosed to the patient's own context, and recalibrated on a defined cadence. The safety conversation has three dominant vectors. First, candidacy: cardiovascular history, breast and uterine history, personal or family clotting history, migraine pattern, and current medications shape both whether BHRT is appropriate and which modality fits. Second, molecule and combination: for women with an intact uterus, estradiol is paired with progesterone to protect the endometrium; transdermal delivery is generally preferred over oral in patients with thrombotic risk. Third, oversight: longitudinal biomarker monitoring catches estradiol, progesterone, lipid, and metabolic drift early. Like every hormone pathway, BHRT is not appropriate for every patient, and the assessment is designed to surface that before initiation.
