Insulin resistance develops over time as cells become less responsive to insulin's signal to absorb glucose, forcing the pancreas to produce more insulin to maintain normal blood glucose. It precedes type 2 diabetes by years to decades and drives much of cardiometabolic disease — hypertension, dyslipidemia, NAFLD, PCOS, and elevated cardiovascular risk. Early detection is the entire point: fasting insulin and HOMA-IR detect the resistance phase, while A1C and glucose only rise after pancreatic compensation begins to fail. oMeds provider review uses fasting insulin, HOMA-IR, A1C, fasting glucose, triglyceride/HDL ratio, and ApoB to characterize metabolic flexibility.
