TRT can raise hematocrit; values above the upper reference limit increase viscosity-related risk. The review pairs hematocrit and hemoglobin with ferritin, oxygen saturation, sleep history, hydration, and blood donation history. First-line adjustments are dose reduction, more frequent smaller doses (smoother kinetics), or a modality change. Untreated obstructive sleep apnea is a leading reversible driver.
