My estradiol is high (male) — what should I do?

Short answer

Mildly elevated estradiol on TRT is often physiologic and well tolerated. Symptomatic elevation (gynecomastia, water retention, mood) is reviewed against modality, dose, body composition, and aromatase load. Anastrozole is used sparingly and only by provider review.

On testosterone optimization, some rise in estradiol is expected and protective for bone, lipids, and libido. Symptomatic elevation — gynecomastia, water retention, mood — is reviewed against dose, modality (injection vs cream vs pellet), body composition (adipose aromatase), and concomitant medications. First-line adjustments are dose, frequency, or modality change. Aromatase inhibition (anastrozole) is used sparingly and only by provider review because over-suppression harms bone, lipids, libido, and joints.
