Enclomiphene and TRT both treat low testosterone but work differently. TRT replaces testosterone exogenously (injectable, transdermal, oral) — effective, fast-acting, and well-studied, but typically suppresses endogenous LH and FSH, reducing fertility. Enclomiphene blocks estrogen feedback at the hypothalamus, raising LH and FSH and stimulating the patient's own testicular testosterone production — preserving fertility and testicular volume but with less consistent symptom resolution in some men.
