Erectile dysfunction is multifactorial — endocrine, vascular, neurogenic, psychogenic, medication-related, or any combination. TRT addresses the endocrine vector when testosterone is a documented contributor. In that context, libido and erectile response often improve within the first weeks of optimization as serum testosterone moves into the optimization range. When the dominant vector is vascular, neurogenic, or psychogenic, TRT alone is not sufficient. In those cases a PDE5 inhibitor (tadalafil, sildenafil), behavioral support, or a combined pathway may be the provider-reviewed answer. The optimization assessment is designed to surface which vector is dominant before a pathway is selected, so the patient is not started on the wrong molecule for the wrong cause.
