A fraction of testosterone aromatizes to estradiol through the aromatase enzyme. On TRT, estradiol rises in proportion to dose, frequency, modality, and adipose tissue mass (a major site of aromatase). Some rise is expected and protective for bone, lipids, and libido. Symptomatic elevation is managed first by adjusting dose, frequency, or modality. Aromatase inhibition (anastrozole) is used sparingly and only by provider review because over-suppression harms bone, lipids, libido, and joints.
