The majority of daily testosterone production occurs during sleep, with peak release tied to REM cycles. Sleep restriction below approximately 5 hours per night demonstrably suppresses morning testosterone within a week in healthy young men. Obstructive sleep apnea independently lowers testosterone through hypoxic and HPA-axis effects. Sleep is therefore the foundational lever under any hormone optimization plan. Sleep evaluation (including apnea risk) is part of every assessment, and apnea is referred for evaluation when risk is elevated.
