It binds GHRH receptors on the anterior pituitary and stimulates pulsatile release of endogenous growth hormone, which then drives hepatic IGF-1 production. The IGF-1 rise on sermorelin is typically modest and preserves the pulsatile rhythm of natural GH release, in contrast to direct recombinant GH administration which produces a flatter, suprapharmacologic exposure.
