Prolactin suppresses GnRH at the hypothalamus, lowering LH/FSH and downstream testosterone. Elevated prolactin in men contributes to low libido, erectile changes, and low testosterone; in women, it contributes to menstrual disruption and low libido. The workup repeats prolactin fasted and midmorning, screens medications (antipsychotics, metoclopramide, opiates, some antidepressants), checks TSH, and runs a macroprolactin assay to exclude artifact. Persistent unexplained elevation prompts pituitary MRI.
