Acute alcohol intake transiently suppresses testosterone. Chronic heavy intake lowers total testosterone through testicular toxicity, raises estradiol through hepatic effects, raises SHBG with advanced hepatic disease, and contributes to fatigue, sleep disruption, and weight gain — all of which compound the picture. Reducing alcohol commonly improves baseline testosterone and many of its downstream signals without any pharmacologic intervention. Alcohol history is part of the assessment.
