Common, usually reversible BHRT side effects include breast tenderness, early-cycle irregular spotting, mild mood shifts, fluid retention, and headache. Most are dose-related and resolve with modality, dose, or progesterone-strategy adjustments. Persistent unexplained vaginal bleeding warrants prompt provider review and may require endometrial evaluation. Less common but reviewed: migraine pattern change, gallbladder symptoms (oral estrogens), and rare thrombotic events (transdermal carries lower risk). Every plan is recalibrated against symptoms and biomarkers.
