Absolute contraindications to BHRT include active estrogen-sensitive cancer (breast, endometrial), recent venous thromboembolism or active thromboembolic disease, active hepatic disease, undiagnosed vaginal bleeding, and a history of estrogen-sensitive cancer in selected cases. Relative contraindications and decision points include strong family breast cancer history, migraine with aura, uncontrolled hypertension, and selected cardiovascular contexts.
