Personal history of estrogen-sensitive cancer (breast, endometrial) is generally disqualifying for systemic BHRT; local vaginal estradiol may be reviewed individually in selected oncology-cleared cases. Family history is reviewed individually against age at diagnosis, gene status (BRCA, others), and validated risk models. Transdermal estradiol carries a different risk profile than oral. Progesterone choice (micronized vs synthetic) affects breast-related risk signals in the literature. Every decision is synchronous-video reviewed and may involve oncology consultation.
