Tadalafil and sildenafil are PDE5 inhibitors used for erectile changes with a vascular pattern. Candidacy requires no concurrent nitrate or guanylate cyclase stimulator use, acceptable cardiovascular candidacy (no recent MI, unstable angina, severe hypotension), and absence of severe hepatic dysfunction. Drug interactions with alpha-blockers and certain antihypertensives are reviewed. Provider review selects molecule and dosing pattern (on-demand vs daily low-dose tadalafil). Cardiovascular workup is referred when warranted independent of PDE5 therapy.
