The decision integrates symptoms (vasomotor, sleep, mood, cognition, libido, genitourinary), biomarkers, time since menopause onset (the 'window of opportunity'), personal/family breast cancer history, cardiovascular history, and venous thromboembolism risk. Disqualifying conditions include active estrogen-sensitive cancer, recent thromboembolism, active liver disease, and undiagnosed vaginal bleeding.
